I sent an email to the Tube Toolbox support and I'll post it here.
The friend requests aren't sending because Youtube might have possibly changed something.

I hope you can come up with an update soon!

YouTube made changes to their site which broke Tube Toolbox (this happens time to time). I put out 2 new versions of Tube Toolbox that should fix all the problems. Let me know if not.
